INCIDENT REPORT SHEET
Part I: Aspin the Rabbit 

A pet rabbit stopped eating, became very irritable and had trouble hopping on both legs during the renovation.
 
Directions: Please PRINT the following questions.  After composing your answers, please EMAIL them to your President and CEO directly. Emails for each company are listed below. A rubric for Section A will be posted after July 7th.

Please note that while these incidence sections are completed independently, feel free to consult with your fellow team members as well as your President and CEO.

Please include your First and Last name in the body of the email.  

Answer the following questions:

1. What is the connection of observations of the symptoms of the rabbit to exposure to lead?

2. What are the human/animal activities and environmental conditions connecting exposure to lead to the observed symptoms?

3. Give a brief analysis of the geographic information supporting the exposure (refer to specific urls if necessary).

4. Give a brief analysis of the available data (soil, blood, dust, water, sediment etc.) supporting the exposure.

5. Briefly summarize your conclusions about the probable causes.
